Healthier SG is a new era of healthcare in Singapore where primary prevention and chronic disease management taking primary focus. The focus on reducing diseases and improving health largely lies with primary care.
As such, we want to focus on few chronic diseases and brings up to date knowledge by our speakers on management of chronic conditions in primary care, thereby improving the health status of our patients in the community.
Co-operatives link practices together to pool resources and compete more effectively, providing better health outcomes for our patients and better practice viability.
Primary Care Networks provide practices with pooled resources, administrative and training support and will play a key role in Healthier SG care model for the next few decades.
