Diabetes mellitus, hypertension, hyperlipidemia, asthma and mental health have been plaguing modern society like Singapore for a long time.

It is well known that these behavioural conditions require a lot of attention from the co-ordinators of care also known as General Practitioners.

With the Primary Care Network set up in 2018, there has been a concerted effort to transform care within a select group of motivated GPs. 

Moving forward, the ministry of health is embarking on the next journey on Healthier Sg which encompass all the key component of PCN and enhance some of these components more.

One of which is the subsidies of medicines. 

Especially interesting is this happens around the time where many newer and better medicines are launched to help those affected by these diseases.

This series of talks and discussion will help and enable the attendees to have close interactions with the speakers and subject resource persons in planning and implementing chronic disease management in the era of healthier SG.
