Specialists Shared Care Programme

GP+ hopes to link up specialists with GPs for easy referral of services between both parties. This ensures three important things: patients receive appropriate care; specialists get the appropriate referrals; and all parties involved are updated about patient conditions.

 
 

Who can join the Programme?

  • Specialists (such as cardiologists, general surgeons, ophthalmologists) registered as medical doctors with the Singapore Medical Council;

  • Associate Members of GP+; and

  • Aged 16 and above
